To complete your registration please provide your (PHONE #) (NAME OF CHILD) (AGE OF CHILD) in the registration comment box or send your details by email to info@musicalmunchkins.ca 

To receive class updates please add info@musicalmunchkins.ca to your contact list

Caregiver participation is required in class. Unregistered children require confirmation of attendance and will be charged a fee per class, room permitting. Confirmation of registration will be sent by email. Refunds are not provided for missed classes. A pro-rated refund, less an administration fee of $30.00 per child, will be issued if your written request to withdraw is received before the second class of the term. Musical Munchkins reserves the right to reschedule a class in the event of unforeseen circumstances. I do hereby release and discharge Musical Munchkins Inc., and its employees, from any and all actions, claims and demands for upon or by reason of damage, loss or personal injury which may be sustained by my child(ren), myself, my caregiver, my guest(s) or my property during the course of or as a result of this musical activity. By accepting this registration I acknowledge that I have read and understand the program policies of Musical Munchkins Inc., and agree to all terms and conditions. 

In no event shall the service provider be liable for any damage arising, directly or indirectly, from the use of the information contained on this web site including damages arising from inaccuracies, omissions or errors. Any person relying on any of the information contained on this web site or making any use of the information contained herein, shall do so at its own risk. The service provider hereby disclaims any liability and shall not be held liable for any damages including, without limitation, direct, indirect or consequential damages including loss of revenue, loss of profit, loss of opportunity or other loss.
